The Cape and Islands United Way has awarded Community Health Center of Cape Cod a $10,000 grant in support of their Caring for Patients with Complex Care Needs program that helps patients who have complex needs and multiple co-morbidities, including patients living with diabetes.
The Complex Care team works with patients’ primary care providers to devise a wellness plan that may incorporate dietary changes, exercise, education, and guidance.
